It looks like the Tomberlin Vanish part number 4001699 maps to the following brands as all being equivalent:

NTN 6206DU8A
NSK 6206V (Free Cable End)
SKF BMD-6206/064S2/UA008A

Here's a picture (attached) of the NSK part. I've not torn down my motor yet and I don't have a parts diagram that shows it, but I believe the Vanish uses the part without a connector. Can anyone confirm this?

Update on getting the part and replacing it ...

I was able to source the correct sensor bearing with connector for $98.99 with shipping included, payable via PayPal from here:

NSK Sensor Bearing 6206V
Connector Option: AMP 4-Pin Connector

I had to pull all the batteries, disconnect the rear brake cables, disconnect the transmission control cables, seat and side fairings to get to the motor. I had to remove the electric motor from the transmission and remove from the cart. Once on the bench, I removed the armature and then used a puller to get the old bearing off. Reversed everything to put back together. All the battery J hooks were rotted, so fabricated replacements out of threaded rod, which cost ~$20.

It now works again at full speed, so I'm very happy with the result.
